  • Ultrasound Technologist - PRN (as needed)

    Job Summary:  The Ultrasound Technologist is responsible for the independent operation of sonographic equipment, and for performing and communicating results of diagnostic examinations using sonography. The Ultrasound Technologist is also responsible for daily operations of the sonographic laboratory, patient schedule, equipment maintenance, the report of equipment failures, and quality assessment (QA). The Ultrasound Technologist maintains a high standard of medical ethics at all times and is self-motivated to increase level of understanding and knowledge of the field, disease and new procedures as they evolve.

    Qualifications:

    1. Satisfactory completion  of  formal  Radiologic  technology  training  in  an  AMA-approved  school  and   ability   to   meet   requirements   for   registry   by   the   American   Registry   of   Radiologic Technologists (ARRT) and/or Certified Radiologic Technologist (CRT)
    2. Graduate of a formal Diagnostic Medical Sonography Program that is accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) preferred
    3. Current compliance with Continuing Medical Education (CME) requirements for specialty, as appropriate.
    4. Current BLS
    5. Ability to effectively operate sonographic equipment
    6. Ability to use independent judgment to acquire the optimum diagnostic sonographic information in each examination performed
    7. Ability to communicate effectively with the patient and the health care team, recognizing the special nature of sonographic examinations and patient’s needs
    8. Ability to establish and maintain effective working relationships with the public and health care team
    9. Ability to follow established departmental procedures
    10. Ability to work efficiently and cope with emergency situations

    Lifting Requirements:

    Heavy lifting:  up to 100 lbs. maximum with frequent lifting and/or carrying objects weighing up to 50 lbs.  Must be able to stand, stoop and lift and walk without rest.

    Location/Hours:  Chesterfield, Bennettsville.  This is a PRN position.  The Ultrasound Technologist will work as needed.

    Salary:  This is an hourly position.  $18 – $25 per hour depending on experience.

  • Housekeeper - Part Time - Floater

  • Bilingual Customer Service Representative - Full Time - Hartsville

    POSITION SUMMARY: Responsible for accurate captioning of all charges from the Electronic Medical Record (Ge Centricity/ eClinical Works) to include any CPT, G-codes and ICD-10 codes needed for billing.   • Performance Improvement Assistant - Full Time - Darlington County

    Position Summary:  The PI Assistant helps coordinate the development, implementation, and evaluation of CareSouth Carolina’s (CSC’s) overall performance improvement program.  This includes but is not limited to the following:  1) supporting the corporation’s performance improvement process, 2) identifying performance trends, as well as 3) prioritizing, and recommending improvements.  The PI Assistant also plays a supportive role in ensuring compliance with all Joint Commission on Accreditation of Healthcare Organizations (JCAHO) standards.  The PI Assistant, as needed, will investigate specific areas of concern as identified by the CSC’s leadership.

    • Provide support to Spread Teams as necessary to ensure that the Spread of Innovation occurs in a timely and effective manner

  • Clinical Patient Navigator - Full Time - Hartsville

    POSITION SUMMARY:  The Clinical Patient Navigator-PrEP/HCV position will assist medical providers in the administration of patient care in assigned clinical areas within the scope of his/her training.  Their primary role is to provide care/case management services in the support of patient care delivered inside and outside of our medical centers. Performs general administrative, clerical and patient interaction duties in accordance with the Joint Commission, CLIA, NCQA and government standards, as well as company policies and procedures.  Function under the direct clinical supervision of the Care Innovations Director to promote optimum patient care and satisfaction while enhancing provider and department effectiveness.  Serve as “communication liaison” between the provider and patient.  Perform within legal and ethical boundaries.  Responsibilities will include:

    •    Manage the flow of patients using the EMR and/or paperwork through the department by interviewing patients and provide case management services to our patients. The employee will use and access the EMR as needed to access the electronic chart. 
    •    Care/case Management services will include but are not limited to referral appointment, answering questions from the patients regarding, screenings and preventive  care, assistance with area agencies providing diet and nutrition services, questions about referral lab tests, setting and motivational interviewing for self-selection of self-management goals, and other case management services as needed. Will go on home visits when needed.
    •       Chronic Disease Care:  Patients with a diagnosed chronic disease will be treated in accordance with established CareSouth Carolina procedures.  Measures: Risk Assessment, Appropriate Therapy, Depression Current PHQ, Depression Appropriate Therapy, HIV/HCV Tests, and any other current Uniform Data Set measures.
    •      Answer patient's questions concerning medication therapies prescribed by our medical providers.
    •      Prevention Activities:  Patients will receive appropriate immunizations, education (within the scope of practice and only under the direction of supervising provider in health maintenance, disease prevention, and compliance with patient’s treatment plan), and other related prevention services in a timely, accurate, and patient centered manner.

    JOB DESCRIPTION:  Under general supervision, performs routine maintenance work, minor electrical, plumbing and carpentry work at all sites.  Conducts safety inspections and makes recommendations for corrective action and follow-up maintenance.  Duties include but are not limited to:

    CareSouth Carolina is seeking the right individuals to improve access to health care services for people living with diabetes or those at risk for diabetes and other chronic diseases. CHWs will seek to bridge any gaps between CareSouth Carolina patients, community members and CSC care teams to enhance communication between people living with diabetes and those providing diabetic care.  CHWs will use motivational interviewing, health education material and evidence-based programs to provide education related to chronic disease management, self-management goal setting, prevention, nutrition and other health and wellness lifestyle changes. In addition, the CHWs through motivational interviewing will work with patients to identify and address Social Barriers to Health (SBTH) such as transportation, food and housing insecurity barriers.  The CHWs will provide home visitation and social service support/navigation. CHWs will work alongside the clinical care teams to identify, establish and facilitate utilization of the medical home and track progress of patient health outcomes.  CHW will assist with eligibility efforts for the Health Insurance Marketplace, Medicaid and other third party insurance needs. The CHWs will assist CWsCHWs assistance in the development of partnerships to improve individual and/or community health.  The ideal CHWs will be a trusted members of the community CSC serves assist in helping to reduce health disparities in underserved communities. Experience related to diabetes management and prevention or the care of individuals living with diabetes or other chronic diseases is plus.

    Key Duties/Responsibilities:

    • Responsible for establishing trusting relationships with patients, families and community.
    • Provide direct services to include: phone calls, home visits, disease self-management, health education and coaching, provider and social support accompaniments; arrange transportation services as needed. Coordinate appropriate referrals for services to community agencies.
