  • Floor Technician - Society Hill

    Floor Technicians are responsible for the overall floor maintenance of hard surface and carpet (stripping, waxing, buffing, shampooing, extraction & bonneting) of  areas that include offices, patient rooms, break rooms, bathrooms, corridors, lobbies, entrances, and other public areas.  Other required duties may be assigned by the supervisor.

    SKILLS/EDUCATION:  Minimum 1 ½ years experience in floor care, knowledge of how to strip, wax and burnish floors with no supervision; knowledge of operating various pieces of equipment necessary to effectively perform job functions, such as a high speed buffer, extractor, wet vac, pressure washer, auto scrubber. Candidate must have a clean 3-year driving record and provide SCDMV record.

  • Family Care Giver Intake Specialist - Full Time - Vantage Point

    Under the direction and supervision of the Family Caregiver Advocate and the AAA Director, the Family Caregiver Intake Specialist will perform telephone and possibly in person interviews with potential clients. They will maintain an adequate system for record keeping of clients served and unmet needs. They will determine the client’s needs and assign them to the appropriate program.  All clients’ information will be entered into the Access System.

  • Clinical Patient Navigator - Full Time - Hartsville

    POSITION SUMMARY: The Clinical Patient Navigator-PrEP/HCV position will assist medical providers in the administration of patient care in assigned clinical areas within the scope of his/her training. Their primary role is to provide care/case management services in the support of patient care delivered inside and outside of our medical centers. Performs general administrative, clerical and patient interaction duties in accordance with the Joint Commission, CLIA, NCQA and government standards, as well as company policies and procedures. Function under the direct clinical supervision of the Care Innovations Director to promote optimum patient care and satisfaction while enhancing provider and department effectiveness. Serve as “communication liaison” between the provider and patient. Perform within legal and ethical boundaries. Responsibilities will include:

    • Manage the flow of patients using the EMR and/or paperwork through the department by interviewing patients and provide case management services to our patients. The employee will use and access the EMR as needed to access the electronic chart.
    • Care/case Management services will include but are not limited to referral appointment, answering questions from the patients regarding, screenings and preventive care, assitance with area agencies providing diet and nutrition services, questions about referral lab tests, setting and motivational interviewing for self-selection of self-management goals, and other case management services as needed. Will go on home visits when needed.
    • Chronic Disease Care:  Patients with a diagnosed chronic disease will be treated in accordance with established CareSouth Carolina procedures.  Measures: Risk Assessment, Appropriate Therapy, Depression Current PHQ, Depression Appropriate Therapy, HIV/HCV Tests, and any other current Uniform Data Set measures.
    • Answer patient's questions concerning medication therapies prescribed by our medical providers.

  • Accounts Payable/Procurement Manager - Full Time - Hartsville

    POSITION SUMMARY:

    The responsibilities of the Accounts Payable/Procurement Manager include but are not limited to:

    Job Summary:  The Ultrasound Technologist is responsible for the independent operation of sonographic equipment, and for performing and communicating results of diagnostic examinations using sonography.
