Community Outreach

CareSouth Carolina takes its role in the community to heart. This is our home, and we understand that the health of any one person is inextricably linked to the health and well-being of the community.

We have a long-standing community health program that engages people in actions that improve personal health and the health of the communities where we live.

CareSouth Carolina has staff in place that can provide one-on-one case management and assistance to assist them with addressing social needs of everything from assisting with signing up for an insurance plan like Medicaid and the Affordable Care Act to assisting with connecting patients with needed services and resources.

Provide health education related to disease prevention, screenings, and healthy behaviors. Examples of health education topics include:

  • Chronic disease prevention
  • Nutrition
  • Physical activity
  • Smoking cessation

Transportation Services: We understand that the lack of transportation is sometimes a barrier to needed care, so we can help facilitate and/or provide limited transportation services for clients/patients.

Translation/Interpretation services: CareSouth Carolina provides language interpretation services to our limited English speaking patients. CareSouth Carolina also arranges for American Sign Language (ASL) services for our patients as well.

OUTREACH SERVICES

Whatever the need, the outreach team is there to help with everything from planning a health fair to helping a friend get into care. You may reach an outreach person by calling any of our center locations and asking for the outreach worker.

  • Facilitate community input through focus groups, public forums and surveys – we listen to our friends and neighbors
  • Research and compile needs and resource assessment data
  • Develop and sustain partnerships and coalitions that, together, build a better, healthier community
  • Facilitate the development and execution of community programs that improve the health of the community
  • Develop the resources, monetary and in-kind, needed to do good in the community
  • Assisting with getting access to care and services
  • Health Screening
  • Blood Pressure
  • Cholesterol
  • Glucose (Blood Sugar)
  • Weight /BMI
  • Community Education (Speaker)
