FAMILY PRACTICE PHYSICIAN - PART TIME OR FULL TIME

POSITION SUMMARY:  The family practice physician provides primary care in a Family Practice clinic setting.  Ambulatory services only, no E.R., in-patient or on-call required.  This position is part or full time with exempt status.  CareSouth Carolina is Federally Qualified Health Center (FQHC) and a Patient Centered Health Home and all employees are an integral part of this model of care delivery.

LOCATION:  Positions available in Hartsville, SC and Bennettsville, SC

DUTIES & RESPONSIBILITIES:
• Provides primary care to children & adults, with emphasis on healing maintenance & preventive care
• Performs charting
• Assists in chronic disease and health management programs
• Performs other duties that may be required to insure the effective operation of the clinic
• Maintain strictest patient confidentiality, except when mandated reporting is required
• Participate as a proactive representative of the Patient Centered Health Home

REQUIRED QUALIFICATIONS, KNOWLEDGE & ABILITIES:

Family Support Services III (CMA/RMA) - Full time - Cheraw

The FSS CMA/RMA will assist medical providers in the administration of patient care in assigned clinical areas within the scope of his/her training.  Their primary role is to provide care management services in the support of patient care delivered in our medical centers. Blended Assistant - Full time - Hartsville

Functioning under the clinical guidance of the Director of Health Services, the role of the Blended Assistant (CMA/Scribe) is to accompany provider into the patient examination room in order to transcribe a history and physical exam, and document accurately the provider’s encounter with the patient and others present in the electronic health record.  He/she will be responsible to list all proper diagnoses and symptoms as well as follow up instructions and prescriptions as dictated by the provider and document any procedures performed by the physician. FLOOR TECHNICIAN - SOCIETY HILL

Floor Technicians are responsible for the overall floor maintenance of hard surface and carpet (stripping, waxing, buffing, shampooing, extraction & bonneting) of  areas that include offices, patient rooms, break rooms, bathrooms, corridors, lobbies, entrances, and other public areas.  Other required duties may be assigned by the supervisor.

SKILLS/EDUCATION:  Minimum 1 ½ years experience in floor care, knowledge of how to strip, wax and burnish floors with no supervision; knowledge of operating various pieces of equipment necessary to effectively perform job functions, such as a high speed buffer, extractor, wet vac, pressure washer, auto scrubber. Candidate must have a clean 3-year driving record and provide SCDMV record.

MAINTENANCE TECHNICIAN - Full Time - Society Hill

JOB DESCRIPTION:  Under general supervision, performs routine maintenance work, minor electrical, plumbing and carpentry work at all sites.  Conducts safety inspections and makes recommendations for corrective action and follow-up maintenance.  Duties include but are not limited to:

  • Maintains buildings, performing minor and routine painting, plumbing, electrical wiring and other related maintenance activities.  Replaces air conditioning filters.

Director of Care Innovations - Full Time - Hartsville

JOB SUMMARY:  The Care Innovations (Ryan White) Director will coordinate the activities of a comprehensive primary health care program focused on the needs of clients with HIV or AIDS in rural Pee Dee communities, including primary medical care, behavioral health care, case management and coordinating a primary care referral network that includes laboratory, pharmaceuticals, oral health, nutrition and other specialty care specific to the needs of HIV and AIDS clients.   The Director is also responsible for the implementation of HIV/AIDS standards of care into the primary care setting, operation of performance improvement programs, audits of medical records and other clinical documentation, management of staff members, oversight of fiscal operations, production of fiscal and operational reports, arrangement of pertinent staff training, oversight of computer database, and overall day to day operations of program.
