This position requires knowledge and a comfort level with helping clients access health services in the primary care setting. In general, the Community Health Worker provides activities and programs that improve the health and overall quality of life of community residents by decreasing barriers that may prevent them from accessing health and medical care; bringing educational messages about health and CSC into the community; and engaging residents in improved individual health. Duties include successful medical home and social service referral; home visits, increasing public education and awareness; the accurate reporting of information and data; positive public relations; and the integration of community services into CSC’s current delivery of care.
High school diploma or GED Equivalent required. Certification as a Community Health Worker is preferred. Experience in the health care industry preferred.

Full Time, 40 hours per week. This position may require occasional Saturday work. Frequent travel within the community and to other locations is required.
Proof of full vaccination for COVID-19 must be presented prior to beginning work with CareSouth Carolina.
