Making A Difference One Patient At A Time!

This is a story that was shared by one of our Team Members with Family Support Services.

I have been working with a patient with DM (diabetes) uncontrollable and a host of other conditions. He had an AIC of 14.7 on November 4, 2014. I have been helping him start a food journal, getting him to exercise, spend more time outside, socializing with others, encouraging him to grow his own vegetables, and just being a sounding board and calling him throughout the weeks encouraging him in every small step he takes, in his words “acting like you care a little bit and want me to care and be healthy”. WELL… hard work on his part is paying off. His A1C today was 11.7.  He has worked really hard and this is huge step in the right direction.  I am so proud of all that he has accomplished so far and look forward to helping him in the future.
