CareSouth Carolina is proud to congratulate Jennifer Gillespie, Population Health Specialist, on being named the Champion of Practice Transformation at the South Carolina Primary Health Care Association (SCPHCA) Clinical Network Retreat.
The Champion of Practice Transformation award recognizes healthcare professionals who demonstrate exceptional leadership, innovation, and commitment to improving healthcare delivery. Gillespie received the honor in recognition of her dedication to helping patients achieve better health outcomes while supporting providers in delivering high-quality, patient-centered care.
A member of the CareSouth Carolina team for nearly five years, Gillespie serves as a Population Health Specialist, working closely with providers to improve patient health by focusing on preventive screenings, immunizations, chronic disease management, patient satisfaction, and other quality measures. She also performs diabetic eye exams for patients with diabetes and works to close gaps in care by connecting patients with the services they need.
"Receiving the Champion of Practice Transformation award was a meaningful recognition of my commitment, leadership, and dedication to improving healthcare delivery," Gillespie said. "This award affirmed the value of driving positive, sustainable change and inspired me to continue advancing excellence in healthcare."
Gillespie said the thing she enjoys most about working at CareSouth Carolina is that the organization puts the patient first.
"I enjoy working at CareSouth Carolina because they care about our patients and the communities we serve," Gillespie said. "CareSouth Carolina always gives back to our communities, and I love that. I also love working for CareSouth Carolina because they care not only about the patients but the employees as well. Here at CareSouth Carolina, we take care of everyone regardless of their income, and we have programs that help ensure everyone has access to the care they need."
Population Health Specialists play an important role in helping patients stay healthy by identifying preventive care needs, managing chronic conditions, and supporting providers in meeting nationally recognized quality standards. Their work helps improve health outcomes while ensuring patients receive timely, coordinated care.
