CareSouth Carolina Welcomes Crystal Leonard, PharmD, to Value-Based Care Team

CareSouth Carolina is proud to welcome Crystal Leonard, PharmD to its Value-Based Care Team. 

As a Clinical Pharmacist holding a Doctorate in Pharmacy, Leonard brings her passion for patient education and medication management to a role that goes far beyond the traditional pharmacy counter.

“I’ve always loved working with patients,” Leonard shares. “In the pharmacy, we’re dispensing medication—but in this new role, I get to take that a step further. I get to work directly with providers as a ‘drug expert’ and educate patients from a medication standpoint.”

Leonard’s role includes supporting patients through Annual Wellness Visits, helping with medication adherence, making preventive care referrals, and working closely with the care team to address each patient’s unique needs.

“Our goal is to make sure patients aren’t just taking their medications—but that they understand them, have access to them, and are getting the full picture of care,” Leonard says. “This means collaborating closely with providers, identifying community health barriers, and helping patients stay on track with chronic condition management.”

When she’s not supporting her patients, Leonard enjoys spending time with her family at the beach and ATV riding.
