Job Description

The Healthy Aging Program Manager provides clinical guidance, mentorship, education, and care coordination support for individuals living with dementia and their caregivers. The Program Manager will assist in the oversight and development of dementia-focused services that enhance quality of life, improve access to community resources, strengthen caregiver support, and reduce unnecessary healthcare utilization. This role works collaboratively with interdisciplinary teams to ensure patient-centered, compassionate, and evidence-based care. The duties of the position include:

Clinical and Programmatic Leadership -

  • Provide clinical consultation and support to patients, caregivers, and staff regarding dementia-related concerns, behavioral management, caregiver burden, and safety planning.
  • Mentor and support Care Team Navigators (CTNs) in case management, care coordination, and patient engagement activities.
  • Facilitate interdisciplinary team discussions, case reviews, and quality improvement initiatives.
  • Assess patient and caregiver needs while identifying gaps in care and community support services.
  • Collaborate with healthcare providers, community agencies, and internal teams to improve continuity of care.
  • Ensure compliance with HIPAA, confidentiality standards, and organizational policies.

Administrative Oversight -

  • Assist with program planning, workflow development, and implementation of best practices.
  • Maintain and update community resource databases and referral information.
  • Support the development and review of protocols related to abuse, neglect, suicide risk, caregiver support, and community referrals.
  • Participate in program evaluation, quality improvement activities, and reporting requirements.
  • Maintain accurate and timely documentation in accordance with organizational standards.

Education and Mentorship -

  • Provide ongoing education and mentorship to CTNs and program staff.
  • Assist in the development of dementia-related educational materials and caregiver resources.
  • Educate patients and caregivers on advance care planning, public benefits, long-term care resources, and available community supports.
  • Serve as a subject matter resource regarding dementia care management and behavioral health support services.
Skills/Abilities/Education/Experience
  • Strong leadership, communication, organizational, and problem-solving skills.
  • Ability to work collaboratively within interdisciplinary healthcare teams.
  • Ability to provide culturally competent, patient-centered care to diverse populations.
  • Demonstrated ability to manage complex patient and caregiver situations with professionalism and compassion.
  • Basic computer proficiency and ability to utilize electronic documentation systems.
  • A master’s degree in Social Work from an accredited institution is required.
  • LMSW or LISW licensure through the South Carolina Board of Social Work Examiners is required.

The following experience is strongly preferred:

  • Experience working with older adults and/or individuals with dementia.
  • Experience in care coordination, case management, or behavioral health services.
  • Previous experience mentoring or supervising staff.
  • Experience working within healthcare or integrated care settings.
  • Familiarity with electronic medical record (EMR) systems.
Hours

Monday – Friday; 8:30 AM – 5:00 PM

Location

Primary location will be Society Hill, SC. Will travel to all CSC locations as needed.
