Position Summary
The Dietitian provides nutrition assessment, counseling, and education to patients in a community health setting. This role focuses on disease prevention and management, supporting patients in making sustainable dietary and lifestyle changes. The Dietitian works collaboratively with providers and care teams to improve overall patient health outcomes. The responsibilities of this position include, but are not limited to
- Conduct comprehensive nutrition assessments for patients
- Develop individualized nutrition care plans based on medical conditions and patient goals
- Provide one-on-one counseling and group education sessions
- Support chronic disease management (e.g., diabetes, hypertension, obesity)
- Educate patients on meal planning, food choices, and behavior change strategies
- Collaborate with providers, nurses, and care team members to coordinate care
- Document patient encounters and progress in the medical record
- Participate in community outreach, education programs, and wellness initiatives
- Monitor patient progress and adjust nutrition plans as needed
Education and Experience
- Master’s degree in Nutrition, Dietetics, or related field
- Registered Dietitian (RD/RDN) credential required
- Licensed in the state of South Carolina
- Experience in community health, outpatient, or public health setting preferred
- A valid driver's license is required

Location
The dietitian will be based in Society Hill, SC, and will travel to all CareSouth Carolina offices, including Bishopville, Hartsville, Society Hill, Chesterfield, Cheraw, Bennettsville, McColl, Lake View, Dillon, and Latta.

Monday – Friday, 8:00 am to 5:00
