Job Description
The Expanded Hours Community Health Worker is responsible for greeting patients, answering the telephone, entering and updating patient demographic information, scheduling appointments, verifying insurance, and providing quality customer service to patients. Provides activities and programs that improve the health and overall quality of life of community residents by decreasing barriers that may prevent them from accessing health and medical care; bringing educational messages about health and CSC into the community; and engaging residents in improved individual health. The duties include the following:
- Patient relationships: interacts with patients and employees in a respectful manner consistent with the mission and values of CareSouth Carolina. Measure(s): patient loyalty.
- Reception and Greeting: Perform the reception and greeting of patients and other guests in a professional, respectful, and compassionate manner consistent with the mission and values of CareSouth Carolina. Measure(s): Patient loyalty.
- Answering Phones: answer, route, take messages, and, as necessary, handle phone calls promptly and courteously in accordance with CareSouth Carolina procedures. Measure(s) Efficiency-Patient show rate.
- Patient Registration: perform the patient registration process, including collecting necessary patient payor information, patient demographics, ensuring that the data is placed in ECW in a timely, accurate, and complete manner, as well as printing the super bill consistent with existing CareSouth procedures. Measure(s) site collections per day, demographic sheet, completion reliability audit, and sliding fee.
- Appointment scheduling/rescheduling/recalls: scheduling, rescheduling, and recall appointments for patients in accordance with existing CareSouth Carolina procedures. Measure(s): Access- third next available appointment, efficiency- patient show rate.
- Daily deposit slip (cash): handling and managing cash for patient care services. This includes reconciliation processes consistent with existing CareSouth Carolina procedures. Measure(s): Occurrence reporting.
- Through community events and other direct contacts with individual community members, the coach identifies individuals and their families in need of health care and provides referrals to CareSouth Carolina or another appropriate health care provider. Provides diligent follow-up and tracking of each service referral or service assistance to ensure that the individual has achieved access and services have been received.
- Assists community members in receiving needed services by navigating the service systems and connecting individuals with the right resources; collaborates with other community organizations and programs on behalf of individuals in need of services. To the extent possible, exhaust all available resources to achieve access to medical or other needed services for everyone.
- In collaboration with care teams at CareSouth Carolina centers, the coach provides one-on-one home visits and other community support services to existing CSC patients in need of personal assistance, an additional assessment, or resources.
- Provides public relations by representing CareSouth Carolina in the community as well as assisting with the implementation of programs and services that promote community understanding and identification with CareSouth Carolina and confidence in its services.
- Collects, assimilates, and reports accurate data on health screenings, referrals, survey results, and other information required that relates to his or her performance and the operation of CSC community services.
- Performs lay health care education and one-on-one health coaching with individuals in community settings within the limits of his or her education and/or licensure. This includes techniques and tools that engage individuals in lifestyle change, self-management goals, etc.
- Assists with the development and implementation of all other department goals and objectives as required. Actively participates and is involved in Quality projects, develops PDSAs, and is an employee who promotes change concepts within their position and with other positions as a team member at CSC.
- Completes all assignments in Relias and signs all documents in PowerDMS on time.
- Willingly perform other duties as assigned.
Education and Experience
- High school diploma or GED Equivalent required.
- Certification as a Community Health Worker is preferred
- Experience in the health care industry preferred.
- BLS certification required.
- Front/medical office experience required; clinical/healthcare experience preferred
- Diabetes management and prevention, or experience supporting individuals living with chronic health conditions, is preferred.

Society Hill – Friday, 11:00 AM – 8:00 PM
Chesterfield - Saturday, 9:00 AM – 1:00 PM
