Assessment Specialist

Job Description

Under the supervision of the Area Agency on Aging Director and the guidance of the Assessment Coordinator, the Assessment Specialist will conduct face to face home visits to clients to assess their need for services contracted through the Area Agency on Aging. Assessment Specialist will be traveling 75% of the work week to conduct in-home assessments. The duties include the following:

  • Complete new client assessments within the required time period established by the South Carolina State Office on Aging Policy and Procedure manual.
  • Complete client home assessments when client has a decline in health or has suffered a catastrophic event
  • Complete a reassessment for each client annually prior to the expiration date.
  • Assist clients by completing in-take applications, assessing needs, determining eligibility, by researching available options and directing consumers to appropriate aging and disability services.
  • Gathers pertinent information and provides it to the I&R Specialist for referral.
  • Screen eligible clients for eligibility of various programs and services and make referrals to services as indicated on the Plan of Service.
  • Document actions and provide follow-up activities as needed.
  • Provide information necessary to enable client to be entered into a Case Worthy system.
  • Prioritize and refer clients for services.
  • Attend mandatory meetings and trainings.
  • Maintain continuing education hours.
  • All documents in Relias and PowerDMS are signed off and completed in a timely manner.
  • Willingly performs other duties as assigned

Education and Experience

  • A Bachelor's Degree is required.
  • A minimum of 2 years of experience in a related field that may include social work, public health, nursing or a related field.
  • Ability to access training needs and objectives.
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
  • A valid driver’s license is required.
  • Ability to solve practical problems and deal with a variety of concrete variable situations where only limited standardization exists.
  • Ability to interpret a variety of instructions furnished in written, oral and diagram or schedule form.
  • Ability to work independently and handle problems involving several concrete variables in standardized situations.
  • Dependable transportation. (travel will be reimbursed)

Hours

Full Time, 37.50 hours per week. Monday - Thursday, 8:00 am - 5:00 pm & Friday, 8:00 am - 1:30 pm.
