Provides high-quality dental hygiene within the scope of service consistent with accepted standards of care. Evaluates patients and implements treatment regimens under the supervision of the Dentist. +Patient Advocate – Full Time – LakeView
DESCRIPTION: The Patient Advocate performs activities that improve the health and overall quality of life in CSC communities by improving access to medical care for CSC patients.   To improve their access to care and their health, provide intensive follow-up services to current CSC patients who are in need of prevention or other services to achieve and ensure needed medical visits.  This includes helping identify patients in need of those services; performing patient follow-up procedures (phone calls, letters, working with fellow family support staff for possible home visit, etc.); scheduling appointments; and ensuring patients are able to make those appointments.  Improve access by working to eliminate the barriers to care for individual patients.   This includes helping patients navigate the service systems and connecting individuals to the right resource and collaborating with other community organizations.  To the extent possible, exhaust all available resources to achieve access to medical or other needed services.  Serve as a customer service and public relations liaison, connecting the patient, the community and CSC services.   This includes educating patients about the programs and services provided by CSC and promoting understanding and confidence in our services.  While providing follow-up services, perform lay health care education within the limits of his or her education and/or licensure.  This includes initial techniques and tools that engage individuals in lifestyle change, self-management goals, etc.  Collects, assimilates, and reports accurate data on patient contacts, appointments achieved, , survey results and other information required that relates to his or her performance and the operation of CSC community services.  This includes documenting barriers to care that arise through follow-up activities.   Interact with patients and employees in a professional and respectful manner consistent with the mission and values of CareSouth Carolina.  Collaboration with clinical and other staff to achieve improved patient care & outcomes during the medical visit.
