+Pharmacy Director - Full Time
JOB DESCRIPTION:  The Pharmacy Director is responsible for the oversight of  daily operations of the four, and any future, CareSouth pharmacies, by providing direction to the site Pharmacist In Charge as well as the direction and oversight of the provisions of professional services, including overseeing all medication storage, preparation, and distribution in the pharmacy. +Associate Medical Director - Full Time

Under the direction of the Chief Medical Officer, the Associate Medical Director is the lead clinician in charge of all aspects of medical patient care services provided by CareSouth Carolina, Inc. including dental services. Working from a solutions-oriented approach, the Associate Medical Director monitors clinical performance to ensure medical services and operations are in compliance with all applicable regulatory and licensing agencies. The Associate Medical Director is responsible for implementing, reviewing and developing Clinical protocols, performance objectives, productivity benchmarks, compliance measures, and provider workflow. Also, the position serves as coach/mentor/trainer to staff providers, giving guidance in best practices, troubleshooting of medical services and operations issues in priority of urgency according to CSC policy and FQHC standards. The Associate Medical Director will also represent the Clinic at required meetings with affiliated agencies.

As a licensed MD the Medical Director will also provide direct patient services in addition to their duties in administration as needed such as to provide coverage when another provider is out or patient demand is extremely high.

+Community Outreach Specialist - Society Hill (Special Programs)

JOB DESCRIPTION:  The Outreach Specialist will identify individuals who test positive for HIV and recruit them for Early Intervention Services.  This position will also provide education to community agencies, organizations, schools and churches regarding the Early Intervention Services program, HIV/AIDS prevention, and the need for early intervention services for HIV/AIDS clients.

**CORE PROCESSES:

Represents CSC in the community, assisting with the implementation of programs and services that promote community understanding and identification with CSC and confidence in its services.  Recruits individuals, their partners and family members for services in the Early Intervention Services program.

Identifies and establishes relationships with a broad range of community agencies, organizations schools and churches that would benefit from education or that might provide referrals to the EIS program.

Plans and implements monthly outreach activities in service area.  Provides community based counseling and testing for HIV to identify and refer clients to the Early Intervention Services program.

Provides necessary and accurate documentation of services provided and completes reports in a timely manner.  Collects data as required for program evaluation and clinical quality improvement.

Plans, coordinates and participates in annual HIV/AIDS/HCV related events while developing and using best practices to recruit and engage target populations at high risk for contracting/transmitting, as well as individuals of unknown HIV serostatus.

Collects blood and other laboratory samples for diagnostic procedures consistent with existing CareSouth Carolina procedures.  Measure(s):  Venipuncture Restick and Patient complaints.

Performs & documents all quality control checks on each test and/or reagent per manufacturer, CareSouth Carolina procedure, or other relevant guideline.  Perform periodic maintenance checks per manufacturer any CSC lab equipment being utilized per CareSouth Carolina procedure, or other relevant guideline.

Collaboration with clinical and other staff to achieve improved patient care & outcomes during the medical visit. This collaboration with other staff will strive to improve the patient experience by providing high quality and patient-centered care.  This may involve participation in trainings, team meetings and huddles to facilitate the delivery of patient-centered care in all CareSouth medical center sites.  This also will involve close collaboration with facilities outside of CSC for referral services when patients are in need of these services.  All clinical staff will include the patient in decision making and the plan of care development while been treated at any CSC facility.

Document the results of test results and all other laboratory data in an accurate, complete, timely manner in GE Centricity and all laboratory logs in accordance with CareSouth Carolina procedures and other relevant standards.

Obtain risk factor data for persons tested for HCV antibody.  Identify chronic HCV infected persons (antibody/RNA) who were not previously aware of their infection.  Ensure all HCV antibody positives are tested for HCV RNA.  Make sure all HCV RNA positive individuals receive results, post-test counseling, and are linked to medical care/treatment and preventative services.

Assists with the development and implementation of all other Care Innovations goals as required.  Actively participates in agency staff meetings and serves on agency/or community committees as assigned.

JOB DESCRIPTION:  The Credentialing Specialist will be responsible for managing all aspects of credentialing and privileging processes for all Physicians, Nurse Practitioners, Dentists, Dental Hygienists, Pharmacists and other healthcare providers, as well as credentialing and re-credentialing providers with insurance plans. The Credentialing Specialist is responsible for maintaining clinical privileges for all relevant providers. Credentialing Specialist will ensure licensure, DEA, and board certifications are current and renewed appropriately.  Other duties may be assigned as needed.

QUALIFICATIONS:  High school diploma required.  Bachelor’s degree in a health care field preferred.  One to three years credentialing or related experience required or any combination of education and experience which would provide equivalent background.

HOURS:  This position is full time working Monday – Friday, 8:30 – 5:00.

+Network Director for Northeastern Rural Health Network - Full-time (Grant Position for 3 years) - Cheraw with work in Dillon, Marlboro, and Chesterfield Counties
JOB DESCRIPTION:  The role of the Network Director is to support the Northeastern Rural Health Network (NRHN) with communications, marketing and coordinating Mental Health First Aid classes, day-to-day administration, community engagement, and data analysis. This position requires the ability to plan, implement, and direct programming that meets the needs of the community. The position also requires the ability to effectively communicate and work with a variety of diverse individuals, groups, and organizations.

DUTIES & RESPONSIBILITIES:

  • Coordinates Network meetings by drafting agendas with input from board members, confirming meeting locations and logistics, reminding Network members of meeting times and locations, and completing and sharing meeting minutes.
  • Serves at the primary point of contact for the Network’s participation in the Healthy People, Health Carolinas—a grant program funded by the Duke Endowment. Supports the successful implementation of the Healthy People, Healthy Carolinas grant activities as needed.
  • Supervises two part-time Community Health Workers as they create community resource manuals and address social determinants that can lead to improvement in overall health in the communities served by the Network.
  • Facilitates the marketing and scheduling of Mental Health First Aid classes, including the securing of location, coordination with instructors, and ensuring needed supplies for the course are purchased.
  • Documents and reports activities and deliverables so that they can be verified by the Board of Directors.
  • Complies with policies and procedures of employing organization and the Network.

QUALIFICATIONS:

Minimum requirements: Bachelor’s degree in the field of human services or public health and one year experience in community organization, administration, supervision, and/or healthcare. Valid driver’s license.

Preferred requirements: Master’s degree in the field of human services or public health and three years’ experience in administration, supervision, and/or healthcare. Experience working with executive boards and grant writing.

Must be able to:

  • Communicate effectively and professionally both in writing and orally and deliver presentations.
  • Respond promptly to questions from community organizations and general public with data-informed responses.
  • Coordinates the activities of grant funded programs and maintaining reporting documentation required by funding sources.
  • Understand and implement quality improvement strategies, including plan, do, study, act cycles.
  • Deliver professional supervision to other Network staff.
