FAMILY PRACTICE PHYSICIAN - PART TIME OR FULL TIME

POSITION SUMMARY:  The family practice physician provides primary care in a Family Practice clinic setting.  Ambulatory services only, no E.R., in-patient or on-call required.  This position is part or full time with exempt status.  CareSouth Carolina is Federally Qualified Health Center (FQHC) and a Patient Centered Health Home and all employees are an integral part of this model of care delivery.

ULTRASOUND TECHNOLOGIST - TEMPORARY, FULL-TIME OR PRN - HARTSVILLE

Job Summary:  The Ultrasound Technologist is responsible for the independent operation of sonographic equipment, and for performing and communicating results of diagnostic examinations using sonography. The Ultrasound Technologist is also responsible for daily operations of the sonographic laboratory, patient schedule, equipment maintenance, the report of equipment failures, and quality assessment (QA). The Ultrasound Technologist maintains a high standard of medical ethics at all times and is self-motivated to increase level of understanding and knowledge of the field, disease and new procedures as they evolve.

Qualifications:

  1. Satisfactory completion  of  formal  Radiologic  technology  training  in  an  AMA-approved  school  and   ability   to   meet   requirements   for   registry   by   the   American   Registry   of   Radiologic Technologists (ARRT) and/or Certified Radiologic Technologist (CRT)
  2. Graduate of a formal Diagnostic Medical Sonography Program that is accredited by the Commission on Accreditation of Allied Health Education Programs (CAAHEP) preferred
  3. Current compliance with Continuing Medical Education (CME) requirements for specialty, as appropriate.
  4. Current BLS
  5. Ability to effectively operate sonographic equipment
  6. Ability to use independent judgment to acquire the optimum diagnostic sonographic information in each examination performed
  7. Ability to communicate effectively with the patient and the health care team, recognizing the special nature of sonographic examinations and patient’s needs
  8. Ability to establish and maintain effective working relationships with the public and health care team
  9. Ability to follow established departmental procedures
  10. Ability to work efficiently and cope with emergency situations

Lifting Requirements:

Heavy lifting:  up to 100 lbs. maximum with frequent lifting and/or carrying objects weighing up to 50 lbs.  Must be able to stand, stoop and lift and walk without rest.

Location/Hours:  Hartsville, SC.  This is a PRN position.  The Ultrasound Technologist will work as needed.

Salary:  This is an hourly position.  $18 – $25 per hour depending on experience.

EMR TRAINER - FULL-TIME - SPECIAL PROGRAMS

JOB DESCRIPTION: This position is responsible for training, assisting and supporting the providers and nurses, and may include other staff, in the use of the Electronic Medical Record (EHR) as it pertains to patient care and data collection.  High level knowledge of the EHR (vender is GE Centricity) is to be used with creative thinking and teaching skills in order to work with staff members and teams to identify best application of currently available software to achieve both quality and efficiency in patient care.  The position requires the ability to foster and maintain positive and close working relationships with staff in all departments, and thereby recommends system changes and enhancements to EHR manager and team as well as provide ongoing updates, training, and feedback to provide/clinical staff in regards to the best use of the EHR.

MEDICAL CASE MANAGER - FULL TIME - CARE INNOVATIONS (HARTSVILLE)

JOB DUTIES: Track individuals who have tested positive for HIV and enroll them in early intervention services while working collaboratively with the health care delivery system of the community health center to develop a plan of care for HIV/AIDS patient.  Assist HIV/AIDS/HCV clients in identifying and negotiating any barriers to treatment.  Maintain documentation, reports, and data entry in accordance with HIPPA, Centers for Disease Control (CDC), Department of Health and Environmental Control (DHEC), and CareSouth Carolina guidelines.  Conduct assessments and assist HIV/AIDS clients to develop service plans, access medical care, medications and specialty care.  Analyze risk factors to determine whether populations at risk are being reached and identify barriers and gaps related to adult viral hepatitis.  Assess adherence to medical standards and collaborate with the medical staff to provide effective, quality services for HIV/AIDS/HCV clients.  This position requires the ability to develop and implement plans and goals, quality interaction with other Ryan White Part B, C, and D agencies in the area.  Interact with federal, state and local entities in order to maximize client access to resources.

EDUCATION/SKILLS REQUIRED:  Must possess a minimum of a bachelor’s degree in Public Health, Social Work or health related field, experience in working with vulnerable populations, knowledge of HIV/AIDS/HCV disease and interventions preferred.

POPULATION HEALTH CARE MANAGER (RN) - FULL TIME
