+Family Practice Physician - Part Time or Full Time
POSITION SUMMARY:  The family practice physician provides primary care in a Family Practice clinic setting.  Ambulatory services only, no E.R., in-patient or on-call required.  This position is part or full time with exempt status.  CareSouth Carolina is Federally Qualified Health Center (FQHC) and a Patient Centered Health Home and all employees are an integral part of this model of care delivery.

DUTIES & RESPONSIBILITIES:
• Provides primary care to children & adults, with emphasis on healing maintenance & preventive care

+Associate Medical Director - Full Time
Under the direction of the Chief Medical Officer, the Associate Medical Director is the lead clinician in charge of all aspects of medical patient care services provided by CareSouth Carolina, Inc. including dental services. Working from a solutions-oriented approach, the Associate Medical Director monitors clinical performance to ensure medical services and operations are in compliance with all applicable regulatory and licensing agencies. The Associate Medical Director is responsible for implementing, reviewing and developing Clinical protocols, performance objectives, productivity benchmarks, compliance measures, and provider workflow. Also, the position serves as coach/mentor/trainer to staff providers, giving guidance in best practices, troubleshooting of medical services and operations issues in priority of urgency according to CSC policy and FQHC standards. The Associate Medical Director will also represent the Clinic at required meetings with affiliated agencies.

As a licensed MD the Medical Director will also provide direct patient services in addition to their duties in administration as needed such as to provide coverage when another provider is out or patient demand is extremely high.

Minimum Qualifications

  • MD or DO degree from an accredited medical school.
  • Board certified/eligible.
  • Current State of South Carolina medical license.
  • At least two years’ experience working with indigent populations and community based programs strongly preferred.

+LPN/Callback Nurse - Full-time - Hartsville (Call Center)
JOB DESCRIPTION:  The role of the Callback Nurse will be to provide consistent, timely assistance to the patients of CareSouth via the telephone.  He/she will be located in our Call Center and receive all incoming calls from patients needing nurse telephone assistance.  The Callback Nurse will be an integral part of the care delivery system assisting all CSC locations with the following nursing responsibilities, as well as other  duties as required :  take calls from outside healthcare entities either completing the requests or transferring call to site, assist patients with medical refill requests per standing orders, continually monitor and complete nurse tasks assigned in the EMR system, and  provide constant collaboration with CSC medical sites and providers.  WORK CONDITIONS:  Will be located in a Call Center.  No hands-on patient contact.  While performing the duties of this job, you must be able to sit for extended periods of time and communicate with callers through the use of a headset.  EDUCATION/SKILLS REQUIRED:  Minimum of an Associate’s degree from a two-year college or technical school or Diploma Nursing Program and three years of related clinical experience required.  Must be licensed as an LPN in the state of S.C.  Excellent computer skills required. JOB DESCRIPTIONS:  Floor Technicians are responsible for the overall floor maintenance of hard surface and carpet (stripping, waxing, buffing, shampooing, extraction & bonneting) of  areas that include offices, patient rooms, break rooms, bathrooms, corridors, lobbies, entrances, and other public areas.  Other required duties may be assigned by the supervisor.  SKILLS/EDUCATION:  Minimum 1 ½ years experience in floor care, knowledge of how to stripe, wax and burnish floors with no supervision; knowledge of operating various pieces of equipment necessary to effectively perform job functions, such as a high speed buffer, extractor, wet vac, pressure washer, auto scrubber. +Behavioral Health Nurse Practitioner - Full-time - Chesterfield
JOB DESCRIPTION

The primary focus of the Behavioral Health Nurse Practitioner will be to provide care for patients receiving Medication Assisted Therapy. Care will be provided both face-to-face and remotely via the use of telemedicine.  The BHNP will also assist our Behavioral Health Providers with the prescriptive needs of patients with co-morbid substance abuse and mental health diagnosis.
