What’s the Medicare Prescription Payment Plan?

The Medicare Prescription Payment Plan is a new payment option in the prescription drug law that works with your current drug coverage to help you manage your out-of-pocket costs for drugs covered by your plan by spreading them across the calendar year (January–December).

Starting in 2025, anyone with a Medicare drug plan or Medicare health plan with drug coverage (like a Medicare Advantage Plan with drug coverage) can use this payment option.

All plans offer this payment option, and participation is voluntary.

This payment option might help you manage your monthly expenses, but it doesn’t save you money or lower your drug costs.

Population Health Specialist

Job Description

The Population Health Specialist will be responsible for collecting and reviewing patient population data and analyzing it to provide risk stratification. They will serve as a leader of population health-based teams to ensure metrics are being addressed for the patient population. The Population Health Specialist will serve as the liaison to health plans and healthcare organizations participating in the care of our patients. The goal of the population health management program is to facilitate high-value, patient-centered care that improves timely access to and provision of preventive services and chronic disease treatment. Duties include:

  • Identify and address care gaps within patient populations, especially in patients with chronic illnesses and diseases.
  • Contributes to a positive experience for patients and families through courteous telephone interactions and interview activities, as well as referral to appropriate clinical staff when necessary.
  • Gather and organize patient data to identify patient unmet needs, and engage patients in care and communication between the patient and their care team.
  • Operates systems such as CareSouth Carolina's EMR, health plan portals, and population health analytics tools.
  • Uses data to risk stratify patients and allocate appropriate resources to patients based on risk. Prepares and submits reports and other relevant work products timely and accurately.

Education and Experience

  • A high school diploma or GED equivalent is required. A two-year degree is preferred.
  • Proficient with electronic medical records and have the ability to provide detailed patient charting
  • Medical office experience is strongly preferred
  • Strong interpersonal & communication skills & the ability to work effectively with a wide range of constituencies in a diverse community
  • Proficient in Microsoft Office applications - Word, Excel, PowerPoint
  • Self-directed, flexible, and comfortable working independently
  • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists
  • Ability to interpret a variety of instructions furnished in written, oral, and diagram or schedule form
  • A valid driver's license required
