Job Description
Track individuals who have tested positive for HIV and enroll them in early intervention services while working collaboratively with the health care delivery system of the community health center to develop a plan of care for HIV/AIDS/HCV patients. Assist HIV/AIDS/HCV patients in identifying and negotiating any barriers to treatment. Maintain documentation, reports, and data entry in accordance with HIPAA, Centers for Disease Control (CDC), Department of Health and Environmental Control (DHEC), and CareSouth Carolina guidelines. Conduct assessments and assist HIV/AIDS clients to develop service plans, access medical care, medications and specialist care. Analyze risk factors to determine whether populations at risk are being reached and identify barriers and gaps related to adult viral hepatitis. Assess adherence to medical standards and collaborate with medical staff to provide effective, quality services for agencies in the area. Interact with federal, state and local entities in order to maximize client access to resources. Provide client services related to consistent medical treatment and medication adherence through collaboration and outreach to persons living with HIV/AIDS/HCV. Duties include:
- Accepts referrals for intake into early intervention HIV/AIDS/HCV program for clinical assessment and treatment of psychosocial issues as well as transportation, housing needs, and medication issues that impacts patients’ health and coping strategies.
- Identifies existing community resources and refers patients to resources and programs as needed or desired. Coordinates referrals to internal and external resources designed to assist HIV/AIDS/HCV patients with medical and psychological needs.
- Provides direct services of assessment and planning for psychosocial problems of HIV/AIDS/HCV patients and family. Documents assessments, findings, treatment goals and objectives of HIV/AIDS/HCV patients in appropriate software.
- Collects data as required for HIV/AIDS/HCV program evaluation and continuous quality improvement program.
- Tracks individuals who are enrolled into HIV/AIDS/HCV care and assesses appropriateness of care to established standards. Responsible for providing written reports and data entry as required for documentation and reporting purposes to include developing action plans for improving patient care.
- Plans, coordinates and participates in annual HIV/AIDS/HCV related events while developing and using best practices to recruit and engage target populations at high risk for contracting/transmitting, as well and individuals of unknown HIV serostatus.
- Interacts with patients and employees in a professional and respectful manner consistent with the mission and values of CareSouth Carolina. Collaborate with other staff to achieve improved patient care and outcomes.
- Meets with patients newly diagnosed with HIV/AIDS/HCV to provide peer support and encourage compliance with medical appointments and medication adherence. Make sure home visits and/or hospital visits as necessary.
- Assists with the development and implementation of all other Care Innovations goals as required. Actively participates in agency staff meetings and serves on agency/or community committees as assigned.
Education and Experience
- High School Diploma
- Three (3) to five (5) years experience in field or related field
