Job Description

Under the supervision of the Healthy Kids Project Director, the CHW will provide Medicaid enrollment assistance to individuals and families, including new enrollments and renewals, in the Pee Dee Region and surrounding areas.  In addition to enrollment assistance, the CHW will provide community education, outreach services, and other needed assistance within the community. The CHW will work to establish partnerships with various organizations to strengthen the services offered to clients.  The CHW will document, track, and verify the Medicaid status and enrollment of potential Medicaid enrollees. This position will require travel to various locations within the state. CHW must be able to work independently. 

Education and Experience

• A high school diploma or GED equivalent is required.
• Must be able to use Microsoft Word and Excel on an intermediate level without assistance.
• Will be traveling across the Pee Dee area, so a valid driver’s license is required.
• Must have the ability to work independently and manage time well.

Hours

Monday – Friday, 8:30 am – 5:00 pm. Occasional weekend work is required for health events. This is a full-time, grant-supported position. The permanency of the position long-term is contingent upon the renewal of the grant.
