Job Description

Clinical Patient Navigator-PREP/HCV position will assist medical providers in the administration of patient care in assigned clinical areas within the scope of his/her training. Their primary role is to provide care/case management services in the support of patient care delivered inside and outside of our medical centers. Performs general administrative, clerical and patient interaction duties in accordance with JCAHO, CLIA, NCQA and government standards, as well as company policies and procedures. Function under the direct clinical supervision of the Care Innovations Director to promote optimum patient care and satisfaction while enhancing provider and department effectiveness. Serve as “communication liaison” between the provider and patient. Perform within legal and ethical boundaries.

Education and Experience

  • One to three years community outreach experience required.
  • Certification in CPR is required.
  • Work required the ability to write, read and understand instructions, add, subtract, multiply and divide numbers using whole numbers, common fractions and decimals. Ability to compute rate, ratio and percent and to draw and interpret bar graphs.
  • Work requires the ability to exchange information on factual matters, communicate in a variety of manners with patients, staff, and explain policies of the Centers.
  • Ability to apply commonsense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations. Job requires providing direct patient care to patients of all ages.
  • Bachelor’s Degree
  • One (1) to three (3) years’ experience in field or related field


Monday – Friday, 40 hours per week.
