Dr. Roosevelt G. Daniel, received a Bachelor of
Arts in Biology from Hampton University in Hampton, Virginia and his medical
degree from University of Alabama School of Medicine. Dr. Daniel completed
his residency at Brody School of Medicine (East Carolina University). This
residency was in internal medicine and pediatrics. He is a member of The
North Carolina Pediatric Society and The American Academy of Pediatrics.
Dr. Roosevelt Daniel’s gentle and professional manner provides quality
health care to his patients and their families in Lee County. |

Susan Scouten, APRN-BC, received her Bachelor of
Science in nursing from the University of Cincinnati, and received a Masters
of Community Health Nursing and Certificate of Graduate Studies in Family
Nursing from the University of South Carolina. She has professional
affiliations with American Nurses Association, South Carolina Nurses
Association, American Academy of Nurse Practitioners, and Sigma Theta Tau,
Alpha Xi Chapter. She served as director of a Nurse Practitioner staffed
primary health care center. Scouten has worked extensively in the primary
health care field since 1982, and has brought to CareSouth Carolina the
expertise and compassion that we strive to maintain. |

Dana Lara Wolfe,
Certified Family Nurse Practitioner, received her Bachelor of
Science in Nursing from Olivet Nazarene University and her Master of
Science in Nursing from Michigan State University. Ms. Wolfe has
experience that includes Delivery of care and tatation of charge duties
in medical, surgical and pediatric units; she also offered relief
staffing for in-patient rehabilitation and CCU. She is a member of
Sigma Theta Tau and the American Academy of Nurse Practitioners. She is
well-rounded at using community resources in evaluating patient needs,
initiating referrals and coordinating their care.

Sharon Denise Swinton, LMSW is a licensed
clinical counselor for CareSouth-Carolina at the Hartsville site. Mrs.
Swinton completed her undergraduate degree at Benedict College in the field
of Social Work. She furthered her education in the School of Social Work at
Clark-Atlanta University in Atlanta, Georgia. Her graduate internship was
done at Grady Memorial Hospital in Atlanta where she obtained an interest in
HIV/AIDS population in terms of their treatment and care. Her master's
thesis was dealt with the topic of providing treatment to the HIV/AIDS
population along with ways of fostering prevention. She continues her
interest in the field as a counselor and an advocate for better treatment
services to individuals suffering from AIDS, mental illness, and a variety
of other clinical problems. |

Sue Cutchin, Registered Nurse, Certified Nurse Midwife, received her Certificate in Nurse Midwifery from the University of Mississippi and her Diploma of Nursing from Norfolk General Hospital. She has been providing health care to women in Cheraw and Chesterfield County area for over 20 years. A staple of the community, she has worked as a midwife since 1977 and continues to devote herself to the women she serves at our Cheraw site. In her practice, she has successfully blended her professional experience and expertise with the personal touch and comforts of a midwife. She takes the time to make pregnancy as healthy and comfortable as possible as well as provide healing and preventive health care for women who are not expecting a child.

Dr. Emanuiel Cooper,
received his Bachelor of Science in Chemistry from Johnson C. Smith
University, his Master of Science in Chemistry from Virginia
Commonwealth University, and his Ph.D. in Chemistry from Atlanta
University. He earned his Doctor of Medicine from Eastern Virginia
Medical School. Dr. Cooper served in the United States Army for four
years as an Army Field Artillery Officer. He holds professional
memberships with American Medical Association, American Academy of
Family Physicians, South Carolina Medical Society, American Chemical
Society, and American Red Cross. Dr. Cooper brings to CareSouth 23+
years of combined experience in many broad based competencies. He is a
highly trained medical professional with exceptional leadership skills.

Cooper comes with some exceptional experience and education and more
importantly, a terrific approach to providing health care. He shares

Dr. Joseph V. Acaylar, Family Practice,
received his Bachelors degree in Medical Technology from the University
of Santo Tomas, Philippines. He completed his residency at Grand Forks
Family Medicine Residency Program in North Dakota and is presently board
certified in Family Medicine. Dr. Acaylar’s personality and
professional manner has earned him respect from his patients and their
families in Hartsville. He is a wonderful addition to out CareSouth
Carolina team.

Tona Tedder, APRN, MSN, FNP-BC, received her
Bachelor of Science degree in Nursing and his Master of Science degree in
Nursing from The University of South Carolina in Columbia, South Carolina.
Tona is a member of South Carolina Nurses Association and South Carolina
Advanced Practice Nurses Council. She is certified with American Heart
Association as Pediatric Advanced Life Support Provider, Advanced Cardiac
Life Support Provider and Basic Life Support Healthcare Provider. Tona has
been working in the healthcare profession for over 30 years, providing care
at all ages with an emphasis on health promotion through education and
management of patients daily. She has been responsible for health education
for all patients with emphasis on Hypertension, Diabetes, Congestive Heart
Failure, Sinusitis, URI, Asthma, GERD, Dermatological Problems, Renal
Disease, etc. Tona is admired and respected by all of her patients because
of the quality of care they receive from her. |

Phil Perkins, has a Masters Degree in Community
Counseling from Georgia State University. He received his Bachelor of
Science degree in Counseling Psychology from Presbyterian College. Phil
holds credentials as a Licensed Master of Social Work, a certified Active
Parenting Leader, and certified by the Institute for Reality Therapy. He is
also a trainer in Therapeutic Crisis Intervention. He is affiliated with
The Institute for Control Theory, Reality Therapy and Quality Management,

Paul A. Coward,
received his
Bachelor of Science degree in Biology from Clemson University and his
medical degree from the Medical University of South Carolina. While at MUSC,
he did research on artificial valve replacement. Dr. Coward did his
internship and residency at Akron General Hospital and Akron Children’s
Hospital with most electives spent in cardiology and neonatology. From
1972-1974 he served as a pediatrician in the United States Air Force. He is
a Fellow in American Academy of Pediatrics, Certified by American Board of
Pediatrics. He has organized and chaired the South Carolina Chapter of the
AAP Child and Drug Abuse Committee. Dr. Coward has been caring for children
in Hartsville since 1975. |

Nursing. She is licensed as an advanced practice nurse by the Board of
Nursing in South Carolina and licensed as a registered nurse in Florida and
South Carolina. Her gentle and professional manner as a pediatric nurse
practitioner provides quality, holistic care to patients and their families. |

| Patricia J. Nelson, received her Bachelor of
Science in Social Work from Buffalo State College and her Masters in Social
Work from State University of Buffalo. As a clinical social worker, Patricia
provided therapy for clients suffering from mental illness and /or drup/alcohol
addiction, developed a Mentally Ill and Chemically addicted Tract for the
Inpatient Drug and Alcohol Unit to treat dually diagnosed clients,
co-chaired continuous Quality Improvement committee and quality Assurnace
committee, and provided mental health education for hospital staff. She is
also experienced with providing assessment, prevention and referral services
to children, teens and adults. Patricia has worked in conjunction with
Research Coordinators to screen and offer treatment/referrals to women
identified as victims of Domestic Violence. |

Dr. Stephen Smith, Family Practice, received his medical degree from the Medical College of Virginia and completed his residency at Roanoke Memorial Hospitals. Serving as CareSouth Carolinas medical director since 1994, Dr. Smith has become a consummate provider of geriatric services in Chesterfield and Marlboro Counties, including the strongest nursing home practice in the Pee Dee region. Certified
Family Nurse Practitioner, received her Master of Science on Nursing from
the Medical University of South Carolina, College of Nursing and her
Bachelor of Science from the Medical University of South Carolina, College
of Nursing. Having spent 12 years as a school nurse in the Darlington
County Public Schools and 2 years as a public health nurse for Darlington
County Health Department, she offers medical services to the entire family.
Ms. Coe is ACLS and BCLS certified. She has diligently worked to provide
patients at our Rosa Lee Gerald Center in Society Hill with quality

associate degree in Nursing from Florence-Darlington Technical College, her

Credentialing Center where she is a certified Family Nurse Practitioner, she

Liz Kershner, MSW, LISW, holds credentials as a Licensed Master of Social Work and a Licensed Independent Social Worker. She received her undergraduate and graduate degrees in Social Work from West Virginia University. Since 1991, she has served as the cornerstone of CareSouth
Carolina’s Behavioral Health program. As its director, she has established one
of the best integrated behavioral health and medical health programs in the
state. She has been involved in Social Work for the last twenty–six years and is affiliated with the Academy of Certified Social Workers and the National Association of Social Workers. Recently, her work with clients at CareSouth Carolina has received national acclaim through her involvement in the National Depression Collaborative. |

Dr. Mohamed Noormohideen, Internal Medicine,
received his medical degree from Triunelveli Medical College in Tirunelveli,
Tamilnadu, India. He completed a Residency in Internal Medicine at the
Institute of Internal Medicine at Madras Medical College in Chennai, India
and as well as a Residency in Internal Medicine at Resurrection-West Lake
Hospital in Melrose Park, Ill. In Ill. Dr. Noormohideen received The
Outstanding Resident of the Year in 2005. He is certified with the American
Board of Internal Medicine, and is licensed in the Sate of South Carolina.
He is also a member of American College of Physicians and the American
Medical Association. Dr. Noormohideen has worked in the Bennettsville area

American Board of Pediatrics and licensed with the South Carolina State

also certified in pediatric advanced life support and neonatal advanced life
support and neonatal air transport. Dr. Candela is a member of The American

in the management of the acutely sick newborn and premature infant, and
their neurodevelopmental follow-up. |

Kirksville College of Osteopathic Medicine. He completed his residency at
Capital Regional Medical Center in Jefferson City, Missouri. Dr. Harness is
board certified with Diplomat of the American Osteopathic and also with the
Board of Family Physicians. Other than South Carolina, he also holds a
medical license in the states of Tennessee and Missouri. He was a United
States Air Force flight surgeon for 4 years. Dr. Harness is known as a
caring person that spends quality time with his patients. |

practicum with the Diabetes of America Program. She has also been an
instructor at Lenoir Community College, where she was the recipient of
Excellence in Teaching Award. Causey’s experience helps her access,

Social Work from the University of North Carolina at Pembroke, her Masters
of Public Administration from Central Michigan University in Fayetteville.
She received her Masters Degree in Social Work from the University of South
Carolina. Toni is in the Alpha Kappa Alpha Sorority and has received many
awards, among them are Dean’s List, Chancellor’s List, Who’s Who Among
American Universities & Colleges, University Marshall and Maurice Boden-Holocaust
Memorial Scholarship. Toni had additional training in conflict management,
and Parent education. She is committed to improving culturally competent
methods for building rural minority health. Toni is experienced in managed

Dr. George Moore, Jr., Family Medicine, received
his premedical degree from Wofford College and his Medical Degree from the
Medical University of South Carolina in Charleston. Dr. Moore underwent his
residency at Greenville Memorial Hospital and served with the United States
Navy as Lieutenant Commander of their Medical Core. He has been serving the
McColl area in healthcare since 1973. Dr. Moore is a certified instructor
for ACLS and is ATLS certified. |

Sylvia McKoy, RN, MSN, FNP-BC, has worked in
healthcare for over 25 years. She has been with CareSouth Carolina since
2002, working at our McColl site. Ms. McKoy has experience providing urgent
health care to patients in the Emergency Room setting. She is also
experienced with providing services in Home Health, Child Health, and Family
Planning. Ms. McKoy has demonstrated a compassionate attitude toward her
patients through her knowledge and kindness. |

M. Robins,
D.O., received
a Bachelor of Science in Business Administration from Villanova University,
and her D.O. Degree from Philadelphia College of Osteopathic Medicine from
there doing her Internship and her residency in Family Medicine at Community
General Osteopathic Hospital in Harrisburg, PA. Dr. Robins is a member of
American Osteopathic Association, American College of Soteopathic Family
Physicians, American College of Occupational and Environmental Medicine, and
the American Osteopathic College of Occupational and Preventive Medicine.
She holds certifications with American Board of Osteopathic Family
Physicians, American Society for Colposcopy and Cervical Pathology, Basic
Life Support, Advanced Cardiac Life Support, Pediatric Advances life
Support, Afvanced Trauma Life Support, and Certified Medical Review
Officer. She has experience in several different facets of family medicine
including inpatient and outpatient medical care, ICU privileges,
occupational medicine and urgent care. Our patients in Chesterfield

Brazil. She complete her residency at Greenville Hospital System in
Greenville, South Carolina. She was born in Switzerland and speaks 5
languages, German, Portugese, English, Dutch, and French. Dr. Biester
is a highly trained medical professional with exceptional leadership
skills. Dr. Biester has fit in well in Chesterfield. She is very active
in the community and her patients have come to know her as a caring
person that spends quality time with them.

Cheryl Lynette Watson, has a Bachelor of Arts
Major in Psychology, with a Minor in Africana Studies. She received Masters
of Social Work from Adelphi University. As a clinical social worker, Cheryl
provided educational and advocacy services for patients and their families,
she has provided group, individual, and family counseling services for
school age students. Cheryl has become a asset of our Chesterfield site by
affecting positive change in her patients lives by the application of her
social work skills and experiences. |
